Players describe Guts and Syringes as a lane defense game similar to Plants vs. Zombies, but much less forgiving. The game explains very little, there is no pause or menu, and many players say it relies heavily on luck due to unbalanced enemies and random syringe drops. A single enemy reaching the end ends the level, which can feel punishing, especially across multiple rounds.
