Catatan Si Boy IV adalah film Indonesia yang dirilis pada tahun 1990 yang disutradarai oleh Nasri Cheppy. Boy (Onky Alexander) kembali ke tanah air untuk berlibur dari sekolah di Amerika. Sudah banyak yang berubah, termasuk Vera (Meriam Bellina) yang sibuk dengan kariernya sebagai foto model. Boy merasa disisihkan. Untuk menghibur sahabatnya itu, Abud (Fuad Alkhar) yang bersama Ina (Sisca Yulianti), adik Boy, buka butik di Bali, mengajak pemuda itu berlibur di Pulau Dewata. Di sana, Boy berkenalan dengan Cindy (Paramitha Rusady) dan langsung akrab. Celakanya, Vera juga melakukan pemotretan di Bali dan memergoki Boy ketika berduaan dengan Cindy.
